What You’ll Need to Sign Up:  a completed carrier packet that includes active operating authority (Articles of Incorporation), a qualified drivers form, and references. The packet should also include copies of the CDL for each dispatched driver, a signed W-9 form, and proof of insurance showing $1,000,000 in auto liability coverage and $100,000 in cargo coverage and carrier and dispatcher agreement form.
